Substitute Information-Teacher

SUBSTITUTE TEACHER

1. Bachelors Degree

2. Copy of a current teaching certificate--if not, you need to apply for a

substitute teaching license

Original transcripts showing your Bachelor's Degree will be necessary if you are applying for a substitute teaching license.

3. A current BCI/FBI criminal background check.

4. Complete a PHESC employment application.

5. Complete a school location sheet.

Submit all items to: Perry-Hocking ESC 1605 Airport Road New Lexington, OH 43764

After all items are complete, you will be given information and access to the Smart Find Express substitute employee management system where you can accept substituting jobs.
